New Patient Forms

SAIDA operates an AI-powered front-desk assistant for orthodontic and dental practices. When you book an appointment by phone with a practice served by SAIDA, the confirmation text you receive may link here.

Your new-patient forms are provided by your practice. The practice's front desk sends or hands you their intake paperwork before or at your first visit. If you were expecting a specific form and don't have it, call your practice directly — the number you originally dialed — and the front desk will take care of it.

How appointment texts work

  1. You call, and you're asked first. While booking on the phone, the assistant asks: “Can we use the number you're calling from for confirmations?” Your verbal yes on that call is the only way texting starts — there is no sign-up form and no list.
  2. You get your confirmation. The text confirms the appointment you just booked, with new-patient form information and directions to the office. The first message includes “Reply STOP to opt out, HELP for help.”
  3. That's it. Messages are one-to-one and tied to your appointment — typically 1–3 per booking. No marketing, promotional, or sales messages, ever.

Consent & opt-out

Appointment texts are sent only after you verbally agree on a call you placed, and only to the number you called from. Consent is not a condition of booking an appointment or receiving care — if you decline, you are booked all the same and confirmed by phone.

Message frequency: typically 1–3 messages per booked appointment. Message and data rates may apply.
